1. Especially for owners of Bandung City ID cards
2. 1 Google Form entry for 1 animal
3. 1 KTP can register a maximum of 3 animals
4. The animal to be vaccinated is in good health
5. Age requirements for animals to be vaccinated:
Cat (minimum 4 months old, minimum body weight 2 kg)
Dog (minimum 4 months old)
