2. Minimum high school education/equivalent
3. Educational background in Communication, Public Health, Public Relations, etc
4. Application letter stamped Rp. 10,000.-;
5. Curriculum Vitae;
6. 2 recent color photographs measuring (4 x 6) cm;
7. Legalized photocopy of diploma and transcript;
8. Photocopy of valid Resident Identification Card (Electronic KTP);
9. Photocopy of Birth Certificate:
10. Valid Police Record Certificate (SKCK) from the local police:
11. Photocopy of certificates relating to Population and Family Planning;
12. Health Certificate from a Government Doctor;
13. Photocopy of SIM A which is still active.
14. Statement letter of non-involvement in politics with a stamp of R. 10,000,-
